One of my screw up's was when I was SYSAD (head admin) for the Prime 550 at the UK office of a large consulting engineers. We had our field engineer in doing a PM and he needed a scratch disk and I said oh you can use xxxx and pointed at the sticky label which had all the disk id's on. Turns out that some one had been using this for a big GIS project in Amman and ended up wiping 6 month's work
