I have had this happen, twice, due to medical. I tell them that one was medical and the other was a sabbatical to take time to learn the latest technologies, as I do not want to scare them off with too many medical "vacations". Yes, technically it's lying but it's the only real way I've found to properly navigate what is a ridiculous system.
