The notable exception is when the doctor has repeatedly failed to accurately determine what's wrong. Then you're "allowed" to bring up your own ideas. I can't wait until the majority of work done by doctors is replaced with a small shell script. They will fight VERY hard to stop that from happening, and they're rich. So it will be a tough fight.
