Somewhat related, but am I only one who're annoyed by the "Did you find this information helpful?" form at the end of some help page? I'd usually just click Yes/No, but then some sites forced me to type in the reason. At this point, I'm like "screw this, I'm out" and just close the tab. Wonder how many times this happened to others.
