I've developed a simple migration tool called mgrt [1] to handle database migrations. It takes plain SQL files, and runs them in order, keeping a hash of the contents of the file in the database to ensure that it cannot be modified once run for the first time. I've posted about it here once before [2]. Check it out if you like. Right now it only supports SQLite, MySQL, and PostgreSQL.
