Instead of having concurrent requests wait for the single request to the backend it's almost always better to use stale-while-revalidate (http://tools.ietf.org/html/draft-nottingham-http-stale-while-revalidate-01 http://tools.ietf.org/html/draft-nottingham-http-stale-while...). AFAIK both varnish and squid support this.
