I routinely take PTO for 'mental health days' and have for many years. No, the company I work for doesn't need to know that; just that I need the day off. I am a very firm believer in taking these days off of 'doing nothing' help prevent my own burn out. Highly recommend for others in the office life.
Just take a day off with no responsibilities and nothing planned. Relax, sleep in, enjoy it.
