Yeah requirements were woefully incomplete, testing insufficient and may I add design where one unreliable sensor was wired to one processor which was given great authority over a system to wind in one direction very fast and where manual reversing out of was very slow or impossible when speeds got high (improbable when pitch directs you down). How could anyone believe this system was safe? No testing or requirements needed.
