I highly doubt the unicode consortium has gone awry. I have seen examples and might attribute it to poorly designed or misconfigured CMS/regurgitation systems that don't handle the unicode properly. It could also result in people not knowing what they are doing - and cutting and pasting between editors and saving as the wrong encoding.
