well, i heard in a podcast that surgeons -- not to pick on them but that was the example given -- don't have their own lives on the line and would likely ignore checklists. compared to say pilots. yet, pilots make mistake too. examples were given when they rush and ignore the checklist or the second in command for example. rushing is usually the biggest mistake. and ignoring a lower ranking pilot is another big one.
