  There's two forms on that website. The first [0] is for everyone. The second [1] is for people who were under 18 at that time.   There are links to two different forms on the File A Claim page [1]: A PDF for adults as of 5/13/2017 [2], and one for minors as of 5/13/2017 [3]. It looks like, if your goals are a balance between receiving the settlement, privacy, and effort, the PDF (probably [2] if no minors were involved) is the way to go.
