I'm not sure this is _why_ there is a limit of 1000 characters, but one possible reason would be to limit the amount of unstructured data put into the assessment field. This can be a way to force people to use structured data fields and more generally to avoid excessive verbosity. (TLDR probably applies to doctors too.)
