Is there some income level where everyone can agree that nobody is a "sucker", and you're just rich at that point? I would imagine all but a handful of doctors make less than 2 million dollars (40 times the median household's income), and those that do I'd be fine calling rich. If you create a new tax bracket for the rich at that point, you're losing only 2% of the increased tax income on the 100 millionaires, but not harming the struggling, debt-ridden doctors of the world.
