Ask HN: How do your company manage test cases

So we have > 10k test cases, we built an internal tool to document them. But it is not ideal. Sometimes QA waste time on testing already broken test because they don’t know the current status. Sometimes dev can’t reproduce because the reproducing environment doesn’t match. Any test case management system can do better?
