The two problems present themselves differently enough that it's confusing pilots (runaway trim is continuous trim movement. MCAS is 5 seconds on, 10 second off). This is a systematic training issue. Pilots have been trained to recognize runaway trim but not faulty MCAS. The only real solution is extensive simulator training until pilots reliably diagnose MCAS activations and start the correct checklist, something which Boeing and airlines don't want to pay for.
