Very ISP dependent: both Verizon and T-Mobile detect using TTL. They also have a list of services which will count towards hotspot data.
For Sprint, everything is old and weird... so they just look at ports like it is 12 years ago. (OpenVPN's default port is bad, 443 is good though)
