No, I mean the apparent over-prescription of antibiotics to everyone involved, them taking a leisurely day walking around and shopping (they must not be that sick), and the fact that antiobiotics are commonly available (at least where I live) for a cash price of $1 from multiple grocery/pharmacy chains. Heck, one chain here (meijer) offers FREE antibiotics.
But, as to your point, I believe people should be fully paying for things like this out of pocket. Insurance is not insurance when it covers everything and every dollar. Imagine how much car insurance would cost if it covered every scratch, ding, and scraped wheel without deductible.
