I'll RTFA, but you know what else helps? Changing the physical environment. Getting away from severe, chronic stressors. Getting to a community that provides services and allows for mobility. "By your bootstraps" doesn't work so well, when you don't have boots. Or when someone keeps "stealing them from you" (metaphorically).
