I'm not sure if or when it was fixed but Witcher 3 had a lot of minor bugs that annoyed me and reminded me of mid-90s PC RPG gaming. Unclickable chests, uncompletable quests (because an item did not drop, or an NPC died instantly), and so on. All very minor inconveniences for my playthrough, it mostly just detracted from the immersion before I moved on.
