Apple Watch can be used without iCloud for sure. iCloud Sync for Health data were added in iOS 11, and it can be disabled easily by a toggle. Apple Watch can only be connected to iPhone, though (not iPod touch) so have it sync to a device without LTE baseband radio is not possible.
