As an Australian I'm a huge fan of our PBS. The government negotiats with manufactures on behalf of all Australians, sets the total amount they will pay (manufacturer, wholesaler, and retailer) price, and then sets the amount that the patient contributes. It's not perfect, and there are a few drugs we get a raw deal on, but overall we all get decent access to whatever medication we need without blowing the budget. Healthcare is basically a right here though, so it's hard to compare to the USA.
