Cool, a new layer to go through when something doesn't work well. On the general issue, it's pervasive at private companies too, where an important problem leads hiring ("this new person will be responsible that it doesn't happen again") and procedures centered around it whatever the cost. Then those companies round up and create a standard around it, and compliance jobs, etc.
