In psychological science "personality disorders" are a class of so-called "disorders" that other people believe are bad, but that the person with the "disorder" does not believe is bad. It is controversial to claim that someone with a "personality disorder" needs help. A "personality disorder" doesn't necessarily involve anosognosia, nor is it limited to things that cause conflict with other people like borderline or narcissistic personality disorders.
