One thing that seems to be not really commented on is "Play Testing / Exploratory Testing" ( and semi related, "dog fooding"). This is the manual test process where someone actually uses the software and looks for problems. Problems are then captured as a new automated test case. This is about finding undesirable unforeseen consequences ( sometimes you get desirable ones! though if it is desirable, you will want to capture it in a test also ).
