Agile isn't working, mostly because people focus on the processes. The thing they forget is the tech. If it takes days or weeks to verify that your code works how agile can you be? Or it's impossible to develop anything new because there are so many bugs. Or your tools are worthless and doesn't help you in development. Also they keep forgetting that if your system is developed with waterfall or no development structure it just won't support an agile way of developing. It's Conway's law...
