  I used to work for a NY based startup as their first employee in London. As part of the package they simply put an extra £1000 a month in my wage under the line "item health insurance contribution". At the time I too went to the Dr approx once a year for something routine and of course in the UK there is a completely free at the point of access healthcare i pocketed the fee.
