I'm not convinced that ease of access to illicit opiates is a major driver for addiction. I'm sure there's a bunch of good dope that's cheap and easily found. I'm curious on the differing levels of use/abuse. My gut tells me that Bangladesh opiate consumption OD rates are lower than Appalachian.
