I think there will always be mistakes in hiring. There is no way to tell perfectly from a set of interactions however much one can wish. We are unpredictable, some are great at deception, we have massive mood swings and phases where we are not at our best, we can deteriorate rapidly and get distracted. What is supposed to be a stable reflection is our education degrees and experience and ideally coupled with a few conversations this should be enough. The problem here is companies have convinced themselves they can find the perfect candidate for every hire and pursue this unicorn relentlessly. Even with all the ado they will frequently get it wrong. It would be more mature and productive to loosen up and deal with the risks than subject everyone to the kind of process.
