Genesis (Barry Silbert's exchange) allows OTC ETH buying. They can probably do OTC other things too, as the old adage goes money talks bullsht walks. I love ringing them up and negotiating prices. OTC is the best and fastest (no accounts, you give them your public address and they send you bank wire instructions), but I don't always have the minimum amount. Kraken is also a national currency exchange that has various markets, I think including Ethereum and Monero.
