I think 40+ hours weeks happen only in the US. This is one of the reasons I'd be reluctant to ever work there. It's very hard to program effectively (effective being the keyword) more than 4-5 hours a day for most people. Sometimes you get that day/week when you're inspired and work more, but otherwise it's just pointless, maybe even detrimental, to force yourself.
