I would say it's the complete lack of Software field being treated like any other major profession. Lack of empirical evidence or evidence based design. A mixture self appointed experts with opinions instead of facts. Sales people who over-sell a product when the engineering department is under-staffed. Over optimistic and out of touch management not knowing how to call developers out on their bull-shot.
