Three times I have been in the situation where the interview candidate was not the one answering questions. Twice we hired someone based on the phone interview and it was someone else who showed up. The third time it was someone reading interview answers off a chat client window. I'm mostly annoyed because it reduces the value of the phone screen and requires more face to face interviews.
