Start by figuring out what your goals for the interview are. In most cases, this will come down to information you want to learn about the candidate and information you want the candidate to learn about the company or project.
Once you know what you want to learn from the candidate you can come up with a list of questions. Think hard about them. Will they really give you the information you want? Rank your questions based on how likely you think they are to give you clear information and how important you think that information is. You'll probably want some redundancy in your questions in case you don't have a clear picture based on the first answer.
