A format I use that I have received compliments on is just a 2-3 column table with headers like this: New | Proficient | Expert Or something along those lines. Then list your skills in there and it's super easy for people to very quickly see your skills and how you rate yourself. The New column is a great way to show that you are learning new things on your own. Most recruiters I've showed to it like it.
