Legacy systems that are totally inflexible is the main problem. Much of this stuff was developed in the mid-1960s. Upgrades and standardization are not considered a "profit center" for an already very profitable industry, and the costs would be enormous. The EMV rollout has cost tens of billions and it doesn't even touch the formats.
