Why is the insurance industry tolerating this? Do the not have an incentive to avoid unnecessary procedures?

  The higher the cost of treatments, the higher the costs of premiums...

    Once a premium has been sold, they can be aggressive in trying not to pay for procedures. It's strange that they're so silent on this one.
