It's true that the US has historically been a leader in medical research, but that largely comes from the federal government through the NIH ($32 billion in 2016.)

Pfizer's foreign market profit might be 2% of sale price, and their US profit 98% of sale price. So despite that figure, the majority of their R&D investment might still come from sales in the US.
