I think it is a bit dangerous to consider this a failure of the surveillance state. One fundamental problem is that intelligence agencies are seen as having been given or having taken on the impossible task of preventing any political violence. While it's understandable to think that they have failed at this, given that the task is impossible, it's probably not a good idea to last the failure at their feet.
