Paper. Password Safe has (or had) Bruce Schneier's recommendation, for what that's worth. I think he helped design the original version, years ago. However, I think the key question is, what are you protecting? I doubt Schneier would recommend Password Safe, for example, for nuclear launch codes, but maybe your email password (depending on what's in your email).
