I don't understand how paying directly or not has anything to do with choosing a surgeon. Plently of socialized medicine system you get zero choice. Some 1/2 socialized, you still have to pay if you want a choice in surgeon. I'm not against socialized medicine I asking how do you think not having "market forces" fixes the problem you're suggesting
