If you are power cycling your computer, it is not sufficient to shut it down and restart it from the power button. You must disconnect its power by turning off the PSU or yanking the cable. I suspect a majority of tech people know this but your PSU passively supplies power to a few things on the board, this can be enough for controller errors to persist across a reboot.
